The goal of this study was to determine the prevalence of F. necrophorum and other agents of bacterial pharyngitis in university students aged 15-30 years, as well as compare the clinical presentation of F. necrophorum to streptococcal pharyngitis. F. necrophorum was detected in 20.5% of patients, with group A and group C/G streptococcus found in only 10.3% and 9.0%, respectively. Positive testing for all three bacterial groups was associated with higher Centor scores, supporting the Centor

“F. necrophorum, which only has been recognized as a potential cause of pharyngitis in adolescents and young adults in the past five years, may cause up to 10 percent of sore throat in those 15-24 years of age,” said Robert Centor, M.D., professor of internal medicine, associate dean of medicine at UAB and the paper’s lead author.

In primary care patients with sore throat, Group C streptococcus had a prevalence of 6.1% (95% CI, 3.1%-9.2%), while F necrophorum had a prevalence of 19.4% (95% CI, 14.7%-24.1%).

Among patients with a Centor score of 2 or higher, the probability of strep or F. necrophorum pharyngitis F. necrophorum contains particulary powerful endotoxic lipopolysaccharides in its cell wall and produces a coagulase enzyme that encourages clot formation. Additionally, it produces a variety of exotoxins, including leukocidin, hemolysin, lipase, and cytoplasmic toxin, all of which likely contribute to its pathogenicity.

F. necrophorum pharyngitis can be associated with development of septic thrombophlebitis of the internal jugular vein, known as Lemierre syndrome (Fig. 1.8). Approximately 80% of cases of Lemierre syndrome are due to this bacterium, but the proportion of patients infected or colonized with F. necrophorum who develop pharyngitis and Lemierre syndrome is unknown. The goal of this study was to determine the prevalence of F. necrophorum and other agents of bacterial pharyngitis in university students aged 15-30 years, as well as compare the clinical presentation of F. necrophorum to streptococcal pharyngitis.
